How to Apply for Services with the Knox County Board of DD:
Birth through age 2
To make a referral, please call (614) 656-3322. You can also make a secure online referral at the Ohio Department of Children and Youth Referral Page. You will need the child’s name, date of birth, address, parent’s name, and contact information.
Ages 3 and over
To apply for services for yourself, your child, or a person for whom you are the legal guardian, please complete the secure online request form at the bottom of this page or contact the Intake Coordinator by email at cpayne@knoxdd.com.
After we receive your request, we will send you an application packet by mail or email, based on the preference you select on the request form.
To continue the eligibility process, you will need to return the completed application packet and provide documentation of the individual's qualifying diagnosis of an intellectual disability or developmental disability.

Applying for services is the first step in the eligibility determination process. Please complete the form below to receive our application packet. Please note you must be the person requesting services, the parent or legal guardian of the person needing services. If you are not one of the people listed above, please complete the Referral Form below:
MAKE A REFERRAL FOR A NON-RELATED PERSON:
To make a referral for someone other than yourself, your child, or a person for whom you are the legal guardian, please complete the information below.
Please note that services are voluntary. After we receive this referral, we will contact the individual and/or their family directly. They will need to complete and return the required eligibility paperwork for the application process to continue. Submitting this referral does not obligate them to apply for services, and they may choose not to proceed.
